Download the Getting to Know your Child form to bring with you to orientation or ask the Enrolment Officer to email an online copy to you which will upload direct to your child’s enrolment record. Remember to bring your child’s up to date Immunisation History Statement if it has been updated since you provided it to us, Medical Action Plan (if applicable), family photo and child photo.
